Because prevention works best when it becomes part of everyday practice.

Does this sound familiar?
PLM was developed to help clinicians address exactly these challenges.

Through case studies, practical exercises and real-world examples, you'll learn how to translate evidence into action and support people to make meaningful, sustainable changes to their health and wellbeing.
PLM focuses on implementation, not just theory. Throughout the programme, you'll explore practical approaches that can be adapted and applied in real consultations to support prevention, personalised care and behaviour change.
You'll explore:
Consultation frameworks that support whole-person conversations
Behaviour change approaches that encourage sustainable action
Goal-setting techniques that help people identify achievable next steps
Self-management support for people living with long-term conditions
Whole-person assessment approaches that consider lifestyle and wider influences on health
Prevention-focused conversations that can be integrated into routine care
